My child is physically fit, except as noted on this form, and has my permission to participate in all camp activities. In the event of illness or injury requiring emergency medical attention and I cannot be contacted, I hereby authorize the camp staff to act for me according to their best judgment. My payment for my child's enrollment in the basketball camp is considered a monetary contribution to the College, designated specifically for usage by Mount Aloysius College
